Each client will have a case manager who will design a program to meet his or her needs.
The Cardiac Rehabilitation Team
Cardiac Rehabilitation involves a long-term commitment from the clients, family, and a team of health care providers.
The cardiac rehab team may include Doctors (such as a family doctor, a heart specialist, and a surgeon), Nurse Practitioners, Nurses, Clinical Exercise Physiologists (ACSM), Physical Therapists, Dietitians, and Psychologists. A Case Manager will help track the client's care.
Working with the team is an important part of Cardiac Rehabilitation. The client should share questions and concerns with the team. This will help the clients reach his or her goals.
People of all ages can benefit from Cardiac Rehabilitation. The lifestyle changes made during Cardiac Rehabilitation have few risks. These changes can improve your overall health and prevent future heart problems and even death.
Exercise training as part of Cardiac Rehabilitaion may not be safe for all clients. For example, people who have very high blood pressure or severe heart disease may not be ready to exercise. You Cardiac Rehabilitation Case manager will work closely with your Physician, Nurse Practioner who will assess your readiness and stability for safe exercise. Ask your Physician whether Cardiac Rehabilitation can help you prevent a future heart problem and improve your health.
After Hospitalization, for 8 Weeks
Focuses on individual exercise prescriptions. While the emphasis is on aerobic activity, it may also include strengthening and flexibility. The need for continuous monitoring is decided on an individual basis. Also focuses on family-centred education to improve coronary disease risk factors.
Follows Recovery Program for 4 Months
Exercise and risk factor management plans are guided by clients goals, needs, and capabilities. Participants will be empowered to adjust their lifestyles for optimal health.
Passport to Wellness
Follows Maintenance Program.
This program has been developed in conjunction with the YMCA. Participants may use all YMCA equipment and take advantage of all YMCA programs three days a week. While not participating in Cardiac Rehab, there is a link with the program and friends.
Many clients fully resume their normal activities of work and leisure. Because of improved lifestyle, many clients feel better and may have greater physical capabilities than before their cardiac event.
