The Regional Coordinator acts as a liaison between the discharged client and Cardiac Rehabilitation services. These services can include Cardiac Rehabilitation education and supervised exercise programs where available across North Eastern Ontario. The Regional Coordinator works closely with Family Doctors, Cardiologists, Cardiac Surgeons, Hospital Administrators, Ministry of Health and Long Term Care representatives and Cardiac Care Network staff as required. Our catchement area extends from Parry Sound to Hearst and Wawa to Matawa including Manitoulin Island. Any person who has been a cardiac patient in the SRH can assess the services of Cardiac Rehabilitation.
There are six satellite programs in the North East region, where on site education and supervised exercise are provided. Ongoing Cardiac Rehabilitation services are being developed for clients who have difficulty accessing some of the services available to urban communities. Through various technological tools, such as this website and the Ontario Telemedicine network (OTN), we are able to deliver important medical and educational services.
Referrals are sent to the six satellite programs directly. Letters are sent out to all other clients advising of the opportunities in their area. Clients with a specific diagnosis such as heart failure are contacted directly.
